Justin Cordy had access to suicide materials due to jail supervision gaps

A coroner found Justin Cordy's 2023 suicide was preventable through better prison supervision.

Justin Cordy died by suicide in Canberra jail on February 26, 2023, after supervision gaps gave him access to materials he needed to end his life. Special magistrate Lucinda Wilson said Cordy showed clear distress: he told a mental health nurse he felt suicidal, told his lawyer his 'headspace was just f---ed', and arrived with bleeding wounds from self-harm.

The prison placed him on hourly checks, but staff did not properly check on him, and the hatch to his cell was covered with paper. Cordy also received a shaving razor with blades in his welcome pack.

The coroner said proper observation would have caught him preparing for suicide and allowed staff to stop him. She made several formal recommendations for the prison to improve supervision and control of razors.
