Inquest reveals workplace pressure led to excavator death
An inquest found Jason Koyla James died when he was under pressure to operate an excavator despite lacking experience.
Jason Koyla James, 29, died on July 20, 2022, when an excavator rolled off a railway track during an upgrade between Western Junction and Evandale. An inquest began on September 21, 2026, in Launceston Magistrates Court.
The court heard James was under pressure to finish the job quickly despite his lack of experience with the excavator. After another worker left the site following an argument, James became the only person qualified to operate it.
While attempting to lift trailers off the track, the excavator tipped and fell on him. Multiple safety failures contributed to the accident.
James was inexperienced, the machine operated on a curve with an unlocked pivoting mechanism that reduced stability, and the load exceeded the machine's limit by more than 200 kilograms.
